Only as a rider to your plan. Baylor Scott & White Health Plan's infertility policy (effective May 2026) says most of its plans cover diagnosing the cause of infertility but not treating it, except as a rider to the contract, and that where treatment is a benefit it is limited to IVF. Its 2026 individual PPO and small group POS plans exclude IVF unless a rider covers it, its 2026 individual HMO excludes it outright, and all exclude artificial insemination and fertility drugs. Texas law makes group plans offer IVF coverage, which an employer can turn down.
What Baylor Scott & White Health Plan's documents say
Read on Oct 26, 2026. The words in quotation marks are the source's own.
Only as a rider to your plan
“Most BSWHP Plans provide a benefit for the diagnosis of the cause(s) of infertility, but NOT for infertility treatment except as a rider to the contract.”
“Treatment of infertility is not a benefit for most BSWHP Plans. Where it is a benefit, interventions are limited to In vitro fertilization (IVF).”
Texas law: offered, not required
The policy quotes Texas's rule that group coverage including pregnancy must offer outpatient IVF coverage unless the group contract holder rejects it in writing, and that plans must cover fertility preservation before cancer treatment.
“Unless rejected in writing by the group contract holder, any evidence of coverage (EOC) providing coverage for pregnancy related procedures must offer and make available coverage for outpatient expenses that may arise from in vitro fertilization procedures.”
“A health benefit plan must provide coverage for fertility preservation services to a covered person who will receive a medically necessary treatment for cancer”
Your plan document decides
Without a controlling mandate, benefits are set by your plan's evidence of coverage or summary plan description, which supersedes the policy.
“POLICY: Please review the plan's EOC (Evidence of Coverage) or Summary Plan Description (SPD) for details.”
“In the absence of a controlling federal or state coverage mandate, benefits are ultimately determined by the terms of the applicable benefit plan documents.”
Who counts as infertile
No conception without medical help after 12 months under 35, or six months at 35 or older.
“after unprotected sexual intercourse / failure of egg and sperm contact / fertilization and uterine implantation without medical assistance after 12 months if under age 35, or 6 months if age 35 years or older.”
Fertility preservation before cancer treatment
Freezing eggs or sperm for members facing cancer treatment can be medically necessary, with prior authorization; the benefit does not include hormone drugs or storage.
“BSWHP may consider Fertility Preservation services medically necessary in males and females facing iatrogenic infertility from cancer treatments”
“PRIOR AUTHORIZATION: Required for fertility preservation services. Genetic / genomic testing may require prior authorization.”
“It does not include hormonal therapies / medications or the storage of such unfertilized genetic materials.”

Whether your Baylor Scott & White Health Plan plan covers IVF
The Baylor Scott & White Health Plan policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.
- 1.Your employer's choice
Most people with Baylor Scott & White Health Plan coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.
- 2.Your plan type
A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.
- 3.Your state's law
Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.
