Does Ambetter cover IVF?
What Ambetter's 2026 marketplace contracts say about covering IVF and other fertility treatment, in their own words, with the document each statement is in.

Not on the Ambetter plans we read. Ambetter's 2026 evidences of coverage for Texas, Florida and Georgia limit infertility benefits to diagnosis (and, in Texas and Florida, treating the underlying condition) and say IVF, artificial insemination and the drugs given with them are not covered. All three cover fertility preservation before treatment that may cause infertility. Centene's clinical policy says infertility treatment is covered only when a member's benefit plan specifically provides it, so a plan in a state that requires IVF coverage can differ.
What Ambetter's own documents say
Read on Oct 12, 2026. The words in quotation marks are the source's own.
Not on the individual plans we read
“Infertility treatment is limited to medical services provided to the member which are medically necessary for the diagnosis of infertility. This does not include treatment or surgical procedures for infertility, including artificial insemination, in vitro fertilization, and other types of artificial or surgical means of conception, nor drugs administered in connection with these procedures.”
“Diagnostic infertility services to determine the cause of infertility and treatment are covered only when specific coverage is provided under the terms of a member/enrollee’s benefit plan.”
What these plans do cover
Tests to diagnose infertility, such as diagnostic laparoscopy, endometrial biopsy and semen analysis, and treatment of the medical conditions behind it, such as endometriosis, blocked fallopian tubes and hormone deficiency (Texas contract).
“Infertility treatment is a covered service expense when medical services are provided to the enrollee which are medically necessary for the diagnosis of infertility such as diagnostic laparoscopy, endometrial biopsy and semen analysis. Benefits are included to treat the underlying medical conditions that cause infertility (such as endometriosis, obstructed fallopian tubes and hormone deficiency).”
Infertility drugs
Infertility drugs are excluded from the pharmacy benefit unless the formulary lists them.
“15. For infertility drugs unless otherwise listed on the formulary.”
Fertility preservation
The Texas contract covers collecting and freezing eggs or sperm when a medical treatment will cause infertility, but not storage; coverage may be limited to in-network providers and may need prior authorization.
“Fertility Preservation Medically necessary fertility preservation services for enrollees when a medical treatment will directly or indirectly result in “iatrogenic infertility,” which is an impairment of fertility by surgery, radiation, chemotherapy, or other medical treatment affecting reproductive organs or”
“(Storage is an exclusion.) Coverage may be limited to in-network providers for fertility preservation services unless the issuer does not have an in-network provider with the appropriate training and expertise to meet the needs of the enrollee. Prior authorization and/or referrals may be required.”

Georgia (Ambetter Health Solutions)

How to see what your Ambetter plan covers
A policy or program page is a summary. Your own plan's documents are what it pays by.
- 1Read your own plan's Evidence of Coverage: the infertility paragraph in the covered services section, and the general exclusions list.
- 2Check the formulary for any fertility drug you have been prescribed: infertility drugs are excluded unless it lists them.
- 3Ask Member Services whether fertility preservation needs prior authorization or an in-network provider before a treatment that may cause infertility.
- 4If you are choosing a plan for next year, read its contract before you enroll, and compare it with what your state requires (the state list is on our insurance page).
