Only if your contract includes it. MVP's 2026 infertility policy covers IVF only where the specific contract or benefit includes it, as New York's IVF law requires of many large group plans, after six failed inseminations in a year under 35 (three in six months at 35 or older), up to three IVF cycles in a lifetime, with prior authorization. Its Vermont plans list advanced infertility care and IVF as not covered, and its Medicaid plans do not cover IVF. Self-funded employer plans follow their own plan description.

Which plans have it

Advanced infertility services, IVF included, are available on plans that carry New York's mandated IVF coverage and on self-funded groups that chose to cover IVF; a self-funded plan's summary plan description says whether it does.

Inseminations first

IVF follows six failed intrauterine or artificial inseminations in 12 months under 35, or three in six months at 35 or older. Where the plan has basic infertility benefits, insemination is covered up to nine cycles per pregnancy.

Three IVF cycles, with prior authorization

IVF is limited to three cycles in a lifetime, counted from the stimulation drugs for a fresh transfer or the lining preparation for a frozen one. Requests need the infertility history and treatment plan, and IVF is on MVP's member list of services that need prior authorization.

What decides it

Whether your MVP Health Care plan covers IVF

The MVP Health Care policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.

  1. 1.Your employer's choice

Most people with MVP Health Care coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.

  2. 2.Your plan type

A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.

  3. 3.Your state's law

Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.

What specific employers publish about their fertility benefits, including which insurer or benefit company runs them: companies that cover IVF.

By state

Does MVP Health Care cover IVF in your state?

Each state's insurance law applied to a MVP Health Care plan issued there. A state law reaches plans issued in that state, usually your employer's state, not where the clinic is.

Must cover IVF (1)
  • MVP Health Care in New York

Often, on fully-insured plans: New York requires some fully-insured health plans to cover IVF, and that includes MVP Health Care plans issued in New York unless one of the law's limits or exemptions applies. Large-group fully-insured plans must cover IVF (up to 3 cycles). A self-funded employer plan that MVP Health Care only administers is not bound by state law and covers IVF only if the employer chose to. New York law and clinics

No state law (1)
  • MVP Health Care in Vermont

Not because of state law: Vermont has no law requiring private plans to cover IVF, so IVF under a MVP Health Care plan there depends entirely on what the employer chose (or, for a plan you buy yourself, on the plan's own terms). Vermont law and clinics

How to see what your MVP Health Care plan covers

A policy or program page is a summary. Your own plan's documents are what it pays by.

  1. 1Read your Group or Subscriber Contract for IVF: MVP says its contracts set the limits and that a policy is not a guarantee of coverage.
  2. 2On a self-funded employer plan, read the summary plan description (SPD) for IVF coverage.
  3. 3Plan on prior authorization, with your infertility history and treatment plan, before IVF.
Questions

MVP Health Care and IVF: common questions

Does MVP Health Care cover IVF?

Only if your contract includes it, MVP's 2026 policy says, as New York's IVF law requires of many large group plans: up to three lifetime IVF cycles, after failed inseminations, with prior authorization. Vermont plans and MVP Medicaid plans do not cover IVF.

How many IVF cycles does MVP cover?

Up to three in a lifetime where the contract includes IVF, and fertility drugs for IVF carry the same three-cycle limit.

Does MVP cover IVF for same-sex couples?

MVP's definition of infertility includes anyone unable to establish a pregnancy because of sexual orientation or gender identity, on plans that include the benefit.

Does MVP cover egg freezing?

When a medical treatment may cause infertility, on its New York commercial plans. Not as a routine backup or to get ahead of reproductive aging, and not on its Vermont, Medicaid or Medicare Advantage plans.
