Does CDPHP cover IVF?
What CDPHP (Capital District Physicians' Health Plan, New York) publishes about covering IVF and other fertility treatment, in its own words, with the document each statement is in.

It depends on your plan. CDPHP's 2026 large business PPO and HMO certificates cover three IVF cycles per lifetime, with freezing and storage of eggs, sperm and embryos for IVF, as New York's law requires of large group plans. Its 2026 individual HMO contract, a small group EPO certificate and its Essential Plan contract list IVF under what they do not cover. All of them cover diagnosing and treating infertility short of IVF, and freezing before a treatment that may cause infertility.
What CDPHP's contracts say
Read on Oct 26, 2026. The words in quotation marks are the source's own.
Large business plans: 3 IVF cycles; individual and small group plans: no
“3. Advanced Infertility Services. We Cover the following advanced infertility services: • Three (3) cycles per lifetime of in vitro fertilization; • Cryopreservation and storage of sperm, ova, and embryos in connection with in vitro fertilization.”
“4. Exclusions and Limitations. We do not Cover: o In vitro fertilization;”
“This Certificate of Coverage (“Certificate”) explains the benefits available to You under a Group Contract between CDPHP Universal Benefits, Inc. (hereinafter referred to as “We”, “Us” or “Our”) and the Group listed in the Group Contract.”
Who counts as infertile
Not establishing a pregnancy after 12 months of regular unprotected intercourse or donor insemination, or six months at 35 or older. CDPHP says it does not decide this coverage on age, sex, sexual orientation, marital status or gender identity.
““Infertility” is a disease or condition characterized by the incapacity to impregnate another person or to conceive, defined by the failure to establish a clinical pregnancy after 12 months of regular, unprotected sexual intercourse or therapeutic donor insemination, or after six (6) months of regular, unprotected sexual intercourse or therapeutic donor insemination for a female 35 years of age or older.”
“We will not discriminate based on Your expected length of life, present or predicted disability, degree of medical dependency, perceived quality of life, other health conditions, or based on personal characteristics including age, sex, sexual orientation, marital status or gender identity, when determining coverage under this benefit.”
Basic, then comprehensive care
If basic infertility services do not increase fertility, the plans cover comprehensive services: ovulation induction and monitoring, ultrasound, artificial insemination, hysteroscopy and laparoscopy. Providers must be qualified under ASRM guidelines.
“Comprehensive Infertility Services. If the basic infertility services do not result in increased fertility, We Cover comprehensive infertility services.”
“Comprehensive infertility services include: • Ovulation induction and monitoring; • Pelvic ultra sound; • Artificial insemination; • Hysteroscopy; • Laparoscopy; and • Laparotomy.”
“All services must be provided by Providers who are qualified to provide such services in accordance with the guidelines established and adopted by the American Society for Reproductive Medicine.”
What counts as an IVF cycle
A cycle starts with the drugs for egg retrieval and a fresh transfer, or with the lining preparation for a frozen transfer.
“A “cycle” is all treatment that starts when: preparatory medications are administered for ovarian stimulation for oocyte retrieval with the intent of undergoing in vitro fertilization using a fresh embryo transfer, or medications are administered for endometrial preparation with the intent of undergoing in vitro fertilization using a frozen embryo transfer.”
Fertility preservation
Every contract read covers standard fertility preservation, collecting, preserving and storing eggs and sperm, when a medical treatment may cause infertility; freezing otherwise is excluded outside IVF coverage.
“4. Fertility Preservation Services. We Cover standard fertility preservation services when a medical treatment may directly or indirectly cause iatrogenic infertility. Standard fertility preservation services include the collecting, preserving, and storing of ova and sperm.”
“Cryopreservation and storage of sperm and ova except when performed as fertility preservation services; o Cryopreservation and storage of embryos;”
Not covered
GIFT and ZIFT, a donor's costs and medical expenses, and surrogacy costs that are not otherwise covered.
“5. Exclusions and Limitations. We do not Cover: • Gamete intrafallopian tube transfers or zygote intrafallopian tube transfers;”
“Costs associated with an ovum or sperm donor, including the donor’s medical expenses;”
“Costs for services relating to surrogate motherhood that are not otherwise Covered Services under this Certificate;”
Self-insured employer plans
CDPHP's affiliate CDPHN administers self-insured employer plans, which set their own benefits.

Whether your CDPHP plan covers IVF
The CDPHP policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.
- 1.Your employer's choice
Most people with CDPHP coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.
- 2.Your plan type
A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.
- 3.Your state's law
Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.
What specific employers publish about their fertility benefits, including which insurer or benefit company runs them: companies that cover IVF.
Does CDPHP cover IVF in New York?
New York's insurance law applied to a CDPHP plan issued there. A state law reaches plans issued in that state, usually your employer's state, not where the clinic is.
CDPHP in New York
Often, on fully-insured plans: New York requires some fully-insured health plans to cover IVF, and that includes CDPHP plans issued in New York unless one of the law's limits or exemptions applies. Large-group fully-insured plans must cover IVF (up to 3 cycles). A self-funded employer plan that CDPHP only administers is not bound by state law and covers IVF only if the employer chose to. How to see what your CDPHP plan covers
A policy or program page is a summary. Your own plan's documents are what it pays by.
- 1Read your Certificate of Coverage (or contract) section on infertility treatment: large business certificates include IVF, individual and small group ones exclude it.
- 2Check for amendments, riders or endorsements to your certificate, which can change benefits.
- 3Ask your doctor whether prior authorization applies before treatment, as CDPHP's family-planning guide notes.
