Only if your plan has an infertility benefit. Since January 1, 2026, Oxford uses UnitedHealthcare's commercial medical policies, and the infertility policy (effective June 1, 2026) says certain plans do not cover infertility services and that IVF and other assisted reproduction are excluded unless the member has an infertility benefit. Oxford says your plan document or Certificate of Coverage governs. For members who have the benefit, Optum reviews infertility treatment under its Managed Infertility Program, and prior authorization is required.
What Oxford's documents say

Only if your plan has an infertility benefit
“Effective January 1, 2026, Oxford will utilize the UnitedHealthcare Commercial Medical Policies.”
“Assisted reproductive technologies, ovulation induction, and insemination procedures are excluded from coverage unless the member has a benefit for Infertility and the criteria listed in the Coverage Rationale section have been met”
Your Certificate of Coverage governs
Oxford's policy notice says some policies may not apply to self-funded members and certain insured products, and that your plan document or Certificate of Coverage governs where it differs from a policy. The infertility policy says certain plans do not cover infertility services, and that state mandates and the plan document must be reviewed.
“Certain policies may not be applicable to Self-Funded members and certain insured products. Refer to the member specific benefit plan document or Certificate of Coverage to determine whether coverage is provided or if there are any exclusions or benefit limitations applicable to any of these policies.”
“If there is a difference between any policy and the member specific benefit plan document or Certificate of Coverage, the member specific benefit plan document or Certificate of Coverage will govern.”
“Certain plans do not cover Infertility services. Legislative mandates and the member specific benefit plan document must be reviewed when determining benefit coverage for Infertility services.”
Optum's Managed Infertility Program
Optum reviews infertility services for every Oxford commercial member with an infertility benefit; its infertility nurse case managers help members with treatment decisions and where to get care. Your provider requests prior authorization, and Oxford's prior authorization list includes infertility services.
“Optum, a UnitedHealth Group company, is delegated to perform reviews for infertility services under the Managed Infertility Program (MIP) for all Oxford commercial members with an infertility benefit.”
“Optum infertility nurse case managers provide support and help members make informed decisions about infertility treatment and care through treatment education, considerations in choosing where to obtain care and assistance navigating the health care system.”
“Infertility Prior authorization required.”
Oxford Fertility Solutions
Oxford's fertility program for employers offers a dedicated fertility nurse and a national network of infertility clinics it calls Centers of Excellence.
“A dedicated fertility nurse”
“Our national COE network features leading infertility treatment clinics that help reduce instances of multiple births and preterm deliveries.”
How the policy defines infertility
Not getting pregnant after 12 months of regular unprotected intercourse under 35, or 6 months at 35 or older, or being unable to get pregnant because of medical, sexual or reproductive history.
“Failure to achieve a pregnancy after 12 months or more of regular unprotected sexual intercourse when the female partner is less than 35 years of age.”
“The inability to achieve a successful pregnancy due to an individual’s medical, sexual, or reproductive history.”
Fertility drugs
Fertility drugs given in a hospital, an office or at home fall under the infertility benefit; self-injected fertility drugs follow the plan document and the pharmacy benefit.
“Pharmaceutical products for the treatment of Infertility that are administered on an outpatient basis in a hospital, alternate facility, physician's office, or the home”
“Self-injectable drugs for Infertility (Refer to the exclusion for self-injectable drugs in the member specific benefit plan document; refer to the pharmacy benefit administrator for self-injectable medication benefit information.)”
Fertility preservation and egg freezing
Fertility preservation is a benefit when a medical treatment such as chemotherapy or radiation will cause infertility. Elective egg freezing and embryo banking are excluded, as is storage longer than a year.
“Benefits are available for fertility preservation for medical reasons that cause irreversible Infertility such as chemotherapy, radiation treatment, and bilateral oophorectomy due to cancer; refer to the member specific benefit plan document.”
“In vitro fertilization that is not an assisted reproductive technology for the treatment of Infertility; this would include but is not limited to elective fertility preservation or embryo accumulation/banking”
“Long-term storage (greater than 1 year) of reproductive materials such as sperm, eggs, embryos, ovarian tissue, and testicular tissue”
Not covered
Donor eggs, sperm or embryos; a gestational carrier who is not a member; and infertility treatment after a sterilization.
“Donor services for donor sperm, ovum or oocytes (eggs), or embryos”
“Surrogate parenting: Services and treatments for a gestational carrier of a pregnancy who is not our member and all related services”
“Infertility treatment when the cause of the Infertility was a procedure that produces sterilization, e.g., vasectomy or tubal ligation”
Whether your Oxford Health Plans plan covers IVF
The Oxford Health Plans policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.
- 1.Your employer's choice
Most people with Oxford Health Plans coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.
- 2.Your plan type
A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.
- 3.Your state's law
Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.
What specific employers publish about their fertility benefits, including which insurer or benefit company runs them: companies that cover IVF.
Does Oxford Health Plans cover IVF in your state?

How to see what your Oxford Health Plans plan covers
A policy or program page is a summary. Your own plan's documents are what it pays by.
- 1Read your Certificate of Coverage or plan document for an infertility benefit and its limits: Oxford says it governs.
- 2Ask HR whether your Oxford plan is insured or self-funded: New York, New Jersey and Connecticut IVF laws reach insured plans issued there.
- 3Have your fertility clinic request prior authorization through Optum's Managed Infertility Program before treatment.
