Does EmblemHealth cover IVF?

What EmblemHealth (New York) publishes about covering IVF and other fertility treatment, in its own words, with the document each statement is in.

It depends on your plan. EmblemHealth's commercial infertility guideline (reviewed June 2026) says every large group contract must cover at least three IVF cycles, with the prescription drugs that go with them, under New York's law, and covers unlimited IUI for members who meet the clinical definition of infertility. It also says plan benefits should be checked to make sure advanced infertility services are covered, and that your benefit program governs. New York's IVF law does not reach self-funded employer plans.

Depends on your plan; large group plans cover 3 IVF cycles

“Every large group contract that provides medical, major medical or similar comprehensive-type coverage shall provide coverage for at least three cycles of in-vitro fertilization (IVF) used in the treatment of infertility, including prescription drugs in connection with IVF”

Your benefit program governs

Each benefit program defines what it covers, some exclude services EmblemHealth considers medically necessary, and where the guideline and your benefits differ, your benefits govern. New York's financial regulator says the state's IVF law does not apply to self-funded employer plans.

“Each benefit program defines which services are covered. The conclusion that a particular service or supply is medically necessary does not constitute a representation or warranty that this service or supply is covered and/or paid for by EmblemHealth, as some programs exclude coverage for services or supplies that EmblemHealth considers medically necessary.”

Who qualifies

Not getting pregnant after twelve months of regular unprotected intercourse or donor insemination, or six months at 35 or older. Single women and same-sex couples qualify after three medically managed IUI cycles with donor sperm (the August 2026 revision lowered this from six), and IVF generally follows three failed IUIs unless going straight to IVF is medically indicated.

“defined by the failure to establish a clinical pregnancy after twelve (12) months of regular, unprotected sexual intercourse or therapeutic donor insemination, or after six (6) months of regular, unprotected sexual intercourse or therapeutic donor insemination for a female thirty-five (35) years of age or older.”

Three IVF cycles, single embryo first

A cycle runs from the stimulation drugs for a fresh transfer, or the lining preparation for a frozen one. The first IVF cycle requires a single embryo transfer, and embryo storage ends when the three cycles are used.

“A “cycle” is defined as either all treatment that starts when preparatory medications are administered for ovarian stimulation for oocyte retrieval with the intent of undergoing in- vitro fertilization using a fresh embryo transfer; or medications are administered for endometrial preparation with the intent of undergoing in-vitro fertilization using a frozen embryo transfer.”

Age and ovarian reserve

The guideline removed its age limits tied to FSH levels and IVF cycles in June 2024. Treatment needs at least a 5% chance of conception, and donor eggs are covered until 45 for premature ovarian failure.

Fertility drugs

“Coverage for prescription drugs, if applicable, is limited to medications approved by the federal Food and Drug Administration for use in the diagnosis and treatment of infertility”

Embryo genetic testing (PGT)

Covered as part of IVF when the documented genetic criteria are met; not for choosing an embryo's sex.

“PGT is considered medically necessary as part of an IVF procedure when documentation confirms one of the following:”

Fertility preservation

A separate benefit, outside the three-cycle limit and with no infertility workup, for members facing cancer treatment, planned gender-affirming treatment or another treatment expected to cause permanent infertility. It covers egg retrieval and sperm collection, not creating and freezing embryos. Freezing for other reasons is not covered.

“Fertility preservation services are a separate benefit to preserve fertility when a medical treatment will directly or indirectly result in iatrogenic infertility and do not count towards the three-cycle limit on IVF benefits. No infertility workup is required for coverage.”

What decides it

Whether your EmblemHealth plan covers IVF

The EmblemHealth policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.

  1. 1.Your employer's choice

Most people with EmblemHealth coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.

  2. 2.Your plan type

A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.

  3. 3.Your state's law

Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.

What specific employers publish about their fertility benefits, including which insurer or benefit company runs them: companies that cover IVF.

By state

Does EmblemHealth cover IVF in New York?

New York's insurance law applied to a EmblemHealth plan issued there. A state law reaches plans issued in that state, usually your employer's state, not where the clinic is.

Must cover IVF (1)
  • EmblemHealth in New York

Often, on fully-insured plans: New York requires some fully-insured health plans to cover IVF, and that includes EmblemHealth plans issued in New York unless one of the law's limits or exemptions applies. Large-group fully-insured plans must cover IVF (up to 3 cycles). A self-funded employer plan that EmblemHealth only administers is not bound by state law and covers IVF only if the employer chose to. How to see what your EmblemHealth plan covers

A policy or program page is a summary. Your own plan's documents are what it pays by.

  1. 1Check your plan benefits for comprehensive and advanced infertility services, as EmblemHealth's guideline advises: your benefit program governs.
  2. 2Ask HR whether your employer's plan is a large group, fully insured contract (more than 100 employees): those must cover three IVF cycles under New York law.
  3. 3If the plan is self-funded, ask whether it includes IVF: New York's IVF law does not apply to it.
  4. 4Your doctor sends EmblemHealth the clinical evidence for prior authorization before treatment.
Questions

EmblemHealth and IVF: common questions

Does EmblemHealth cover IVF?

On large group plans, yes: its guideline says every large group contract must cover at least three IVF cycles with the prescription drugs used with them, under New York law. Other plans follow their own benefits, and New York's IVF law does not reach self-funded employer plans.

Does EmblemHealth cover IVF for same-sex couples or single women?

Women without a male partner or exposure to sperm qualify after three medically managed IUI cycles with donor sperm (lowered from six in August 2026). Reciprocal IVF is not covered.

Does EmblemHealth cover egg freezing?

As a separate fertility preservation benefit for members facing cancer treatment, gender-affirming treatment or another treatment expected to cause infertility: egg retrieval and sperm collection, not embryo freezing. Elective freezing is not covered.

How many IVF cycles does EmblemHealth cover?

At least three on a large group contract, with a single embryo transfer required in the first cycle; embryo storage coverage ends when the three cycles are used.
