Does Blue Cross Blue Shield cover IVF?
What the Blue Cross Blue Shield Association and the Blue companies we read publish about covering IVF, each in its own words, with the document each statement is in.

Only if your plan includes it, and the rules differ by Blue company. Blue Cross Blue Shield is a federation of independent companies, each with its own medical policy, and the Association tells members to check their own policy's coverage with their local Blue plan. The policies we read say the same thing in different words: Blue Cross NC covers IVF only if the member contract specifies it, Blue Cross Blue Shield of Michigan only when the employer group added it, Blue Cross and Blue Shield of Texas as a separate plan employers may buy, and Anthem's guideline says some plans exclude or limit infertility treatment. The federal employees' Blue Cross plan has its own rules, on our FEHB page.

Only if your plan includes it
“For details on a provider’s in-network status or your own policy’s coverage, contact your Local Blue Plan and ask your provider before making an appointment.”
“In Vitro Fertilization (IVF): Benefits for IVF are available only if specified in the member contract.”
“Assisted reproductive techniques are only available to members when the employer group has chosen to offer the services as additional/extra benefits, through certificate benefit language or riders.”
Blue Distinction Centers for Fertility Care
The Association's quality designation for fertility clinics, focused on IVF. It is not a statement about coverage: check a clinic's network status and your own policy's coverage with your local Blue plan before an appointment, and each plan may add its own criteria for clinics in its area.
“The Blue Distinction Centers for fertility care program focuses on in vitro fertilization (IVF), which is the most common assisted reproductive technology (ART) treatment.”
“A Local Blue Plan may require additional criteria for providers located in its own service area; for details, contact your Local Blue Plan.”

Anthem Blue Cross and Blue Shield
Depends on your planAnthem's assisted reproduction guideline (published December 18, 2025) says federal and state mandates and plan language override it, and that some plans exclude or limit infertility treatment. Where IVF is a benefit it expects, for example, 3 failed IUI cycles with oral drugs for unexplained infertility, and counts a woman without a male partner as infertile after 12 cycles of supervised IUI under 35, or 6 at 35 and over. Freezing eggs to delay natural aging is not considered appropriate.
“Note: Federal and State mandates, as well as benefit language supersede the content of this document. Some plans may exclude or limit coverage of infertility treatment and other associated services. Please check benefit plan descriptions for details.”
“Failure to have a live birth after 3 cycles of IUI with oral medications for unexplained infertility”
“If under the age of 35 years, the inability to conceive or produce a conception after 12 cycles of documented medically supervised stimulated intrauterine insemination (IUI); or If 35 years of age or older, the inability to conceive or produce a conception after 6 cycles of documented medically supervised stimulated intrauterine insemination (IUI);”
“Cryopreservation is not clinically appropriate when it is done for elective or voluntary sterilization, such as procedures chosen only for birth control, or to delay natural aging.”
Blue Cross and Blue Shield of Texas
A separate plan employers buyTexas law requires insurers to offer separate IVF coverage that employers may buy, and Blue Cross and Blue Shield of Texas sells it that way to fully insured groups. For a group that takes it, the benefit reaches everyone eligible on the policy, same-sex couples and single people included, and requires at least five continuous years of infertility or a listed condition such as endometriosis or blocked fallopian tubes.
“Background: Texas law requires that insurers offer separate plans for employers to purchase if they wish to cover IVF treatment/services.”
“For plans that elect the separate in-vitro coverage offer, this benefit was expanded to all eligible individuals on the policy, including same-sex couples and single individuals.”
“The patient is a covered participant under the plan. The patient has a history of infertility of at least five continuous years OR their infertility is associated with at least one of the following medical conditions: endometriosis, exposure in utero diethylstilbestrol (DES), blockage or surgical removal of one or both fallopian tubes, oligospermia.”
Blue Cross and Blue Shield of Illinois
Depends on your planThe medical policy it shares with its sister plans in Montana, New Mexico, Oklahoma and Texas (effective January 1, 2026) says to check the member's benefit plan first: only if infertility services are a benefit do its medically necessary services apply. It notes that Illinois law requires fertility preservation when a treatment may cause infertility. Illinois's IVF law is in the state section below.
“NOTE 1: Carefully check the member’s benefit plan, summary plan description or contract for language specific to infertility services. IF infertility services are determined to be eligible for member benefits, then the following services that are listed as medically necessary should be considered for benefit coverage.”
“EXCEPTION: Effective for policies issued after January 1, 2019, Illinois law (215 ILCS 5/356z.29) requires coverage and reimbursement for standard fertility preservation services when a necessary medical treatment may directly or indirectly cause iatrogenic infertility to a member.”
Blue Cross and Blue Shield of North Carolina
Only if in your contractIts policy (reviewed March 2026) says IVF benefits are available only if specified in the member contract, and that some plans provide no infertility benefits or exclude insemination and IVF with their drugs and lab work. Everything received as part of an IVF procedure, drugs included, falls under the IVF benefit. Freezing eggs or sperm is not covered unless the benefit booklet says so.
“In Vitro Fertilization (IVF): Benefits for IVF are available only if specified in the member contract.”
“Some plans may provide no benefits. Some plans may provide some coverage, but exclude coverage for artificial means of conception, including, but not limited to artificial insemination, in vitro fertilization and associated services such as drugs, labs, pathology, etc.”
“All services which are received as part of an IVF procedure are considered under the same benefit as the IVF procedure. This can include office visits, drugs, lab and pathology, surgical procedures, etc.”
“Charges related to cryopreservation of reproductive tissue, including sperm and oocytes, are not covered under standard medical benefits or under infertility benefits unless otherwise stated in the member’s benefit booklet.”
Blue Cross Blue Shield of Michigan and Blue Care Network
Only if your employer added itTheir joint policy (effective July 1, 2026) says assisted reproduction, IVF included, is not a general medical benefit: it is available only when the employer group added it through the certificate or a rider, and the employer defines the limits. Treating a medical cause of infertility is covered under basic benefits. For people with cancer, fertility preservation includes storage for up to two years.
“Assisted reproductive techniques are only available to members when the employer group has chosen to offer the services as additional/extra benefits, through certificate benefit language or riders.”
“When infertility is due to an underlying medical condition (e.g., chronic infection, uterine fibroids, etc.), the treatment of that disorder is medically necessary and is covered under basic medical-surgical benefits.*”
“Storage of embryos, mature oocytes and spermatozoa for up to two years”
Florida Blue
Benchmark plan excludes itFlorida's benchmark plan for 2025 to 2027, the plan that sets the state's required benefits, is a Florida Blue small-group plan, and the federal summary of it lists infertility treatment as not covered. That describes one plan, not every Florida Blue plan.
“Infertility Treatment No Not Covered”

Whether your Blue Cross Blue Shield plan covers IVF
Blue Cross Blue Shield's policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.
- 1.Your employer's choice
Most people with Blue Cross Blue Shield coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.
- 2.Your plan type
A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.
- 3.Your state's law
Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.
What specific employers publish about their fertility benefits, including which insurer or benefit company runs them: companies that cover IVF.

How to see what your Blue Cross Blue Shield plan covers
A policy or program page is a summary. Your own plan's documents are what it pays by.
- 1Find the Blue company on your member ID card: each one is a separate company with its own medical policy.
- 2Read the family planning or infertility section of your benefit booklet and its list of what is not covered, or ask your employer for the plan document.
- 3Call the customer service number on your member ID card and ask whether IVF is a covered service on your plan, whether it needs review or approval first, and what the limit is.
- 4If you want a Blue Distinction Center for Fertility Care, confirm with your local Blue plan that the clinic is in network for your plan.
- 5Ask HR whether your plan is fully insured or self-funded: that decides whether your state's law applies (see your state below).
Blue Cross Blue Shield and IVF: common questions
Does Blue Cross Blue Shield cover IVF?
Only if your plan includes it. Blue Cross Blue Shield is a federation of independent companies, each with its own policy, and the policies we read cover IVF only when the member's contract or employer includes it. Call the number on your card and ask whether IVF is a covered service on your plan.
Does Blue Cross Blue Shield cover IVF in Texas?
Only if your employer bought it. Texas law requires insurers to offer separate IVF coverage, and Blue Cross and Blue Shield of Texas sells it as a separate plan to fully insured groups. Where an employer takes it, the benefit requires at least five continuous years of infertility or a listed condition such as endometriosis or blocked fallopian tubes.
Does Blue Cross Blue Shield cover IVF in North Carolina?
Only if your contract says so. Blue Cross and Blue Shield of North Carolina's policy, reviewed March 2026, says IVF benefits are available only if specified in the member contract, and that some plans provide no infertility benefits at all. North Carolina has no law requiring insurers to cover IVF.
Does Blue Cross Blue Shield cover IVF in Michigan?
Only if your employer added it. Blue Cross Blue Shield of Michigan and Blue Care Network's joint policy, effective July 1, 2026, says assisted reproduction is available only when the employer group added it through the certificate or a rider.
Does Blue Cross Blue Shield cover IVF in Illinois?
Often, on fully-insured plans, because Illinois law requires many of them to cover IVF within cycle limits. Blue Cross and Blue Shield of Illinois's medical policy says to check the member's benefit plan first, and a self-funded employer plan follows the employer's choice instead of state law.
Does Anthem cover IVF?
It depends on your plan. Anthem's assisted reproduction guideline, published December 18, 2025, says mandates and plan language override it and that some plans exclude or limit infertility treatment. Where IVF is a benefit, the guideline sets the medical criteria.
Does the federal Blue Cross plan (FEP Blue) cover IVF?
FEP Blue Standard does, up to $25,000 paid a year with prior approval; FEP Blue Focus lists IVF as not covered. The details are on our FEHB page.
