Does Wellmark cover IVF?
What Wellmark Blue Cross and Blue Shield (Iowa and South Dakota) publishes about covering IVF and other fertility treatment, in its own words, with the document each statement is in.

Not on the Wellmark individual plans we read. Its Blue HMO outline of coverage (September 2025) excludes artificial insemination, IVF and any related fertility treatment, and says that once you have one of those procedures, benefits for all fertility treatment, ovulation drugs included, end that day. Fertility tests are covered until then. We could not read a current Wellmark employer plan document or infertility policy, so a plan through work can differ: its own documents decide.
What Wellmark's documents say

Not on the individual plans we read
“The following services are excluded or are not considered medically necessary by Wellmark Health Plan of Iowa and will not be covered”
“Artificial insemination, in vitro fertilization, or any related fertility or infertility treatment or transfer procedure. If you have any of these procedures done, benefits for all types of fertility or infertility treatment (including drug-induced stimulation of ovulation) will end beginning on the day you receive the noncovered service”
Where Wellmark is the Blue plan
Wellmark serves Iowa and South Dakota.
“Wellmark is working hard to provide affordable, accessible and sustainable health insurance solutions while educating and empowering all Iowans and South Dakotans to live long, healthy lives.”
Tests until IVF or insemination
Fertility and infertility services are covered only until you receive artificial insemination, IVF or a related procedure; from that day, benefits for all fertility treatment end, including drugs that stimulate ovulation.
“Fertility and infertility services until you receive artificial insemination, in vitro fertilization, or any related fertility or infertility treatment or transfer procedure”
Also excluded
Donor sperm or eggs, freezing and storing sperm, eggs or embryos, surrogacy, reversing a tubal ligation or vasectomy, and infertility treatment after a voluntary sterilization. The outline names no exception for freezing before cancer treatment.
“The collection or purchase of donor semen (sperm) or oocytes (eggs) when performed in connection with fertility or infertility procedures or for any other reason or service; freezing and storage of sperm, oocytes, or embryos; surrogate parent services”
“Reversal of a tubal ligation (or its equivalent) or vasectomy”
“Infertility treatment if the infertility is the result of voluntary sterilization”
The policy decides
The outline is a summary: only the benefits policy itself determines your benefits. Wellmark's 2026 Iowa summaries of benefits list infertility treatment with the note that some services are excluded.
“Only the actual benefit provisions in your benefits policy will determine your benefits.”
“Infertility treatment (excludes some services)”
Whether your Wellmark plan covers IVF
The Wellmark policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.
- 1.Your employer's choice
Most people with Wellmark coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.
- 2.Your plan type
A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.
- 3.Your state's law
Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.
What specific employers publish about their fertility benefits, including which insurer or benefit company runs them: companies that cover IVF.
Does Wellmark cover IVF in your state?
Each state's insurance law applied to a Wellmark plan issued there. A state law reaches plans issued in that state, usually your employer's state, not where the clinic is.
Wellmark in Iowa
Not because of state law: Iowa has no law requiring private plans to cover IVF, so IVF under a Wellmark plan there depends entirely on what the employer chose (or, for a plan you buy yourself, on the plan's own terms). Iowa law and clinics
Wellmark in South Dakota
Not because of state law: South Dakota has no law requiring private plans to cover IVF, so IVF under a Wellmark plan there depends entirely on what the employer chose (or, for a plan you buy yourself, on the plan's own terms). South Dakota law and clinics

How to see what your Wellmark plan covers
A policy or program page is a summary. Your own plan's documents are what it pays by.
- 1Read your benefits policy: Wellmark says only it determines your benefits, and the outline is a summary.
- 2On a plan through work, ask HR whether the plan adds an infertility or IVF benefit: we could not read a current Wellmark employer document.
- 3Call the member services number on your Wellmark card before starting insemination or IVF, because on the individual Blue HMO plans all fertility benefits end once either is done.
