It depends on your plan. Horizon's own pages say not all of its plans cover infertility benefits and send members to their policy documents or member services. New Jersey requires many fully insured plans to cover IVF, and the state employees' plans Horizon administers follow that law: they cover IVF with up to four completed egg retrievals per lifetime. Horizon's 2026 individual Bronze EPO plan limits infertility treatment to artificial insemination with pre-approval.

“Not all plans cover infertility benefits. Refer to your policy documents or contact Pharmacy Member Services at the toll free number on the back of your Horizon BCBSNJID card for coverage information.”
Fertility drugs come from a specialty pharmacy
To get in-network cost sharing, members must fill certain infertility drugs at a participating specialty pharmacy. Horizon's Fertility Pharmacy Drug Management List (revised May 1, 2026) includes Cetrotide, clomiphene, Follistim, ganirelix, Gonal-f and Menopur, and specialty drugs may need a medical necessity review.
“Horizon BCBSNJ members who use certain prescription drugs associated with the treatment of infertility and want to use their in-network benefits to minimize their cost-sharing, are required to obtain these prescription drugs from one of the participating specialty pharmacies listed below.”
“Fertility and Pregnancy - cetrorelix - CETROTIDE - chorionic gonadotropin - clomiphene citrate - FOLLISTIM AQ - FYREMADEL - GANIRELIX AC - GONAL-F - GONAL-F RFF - leuprolide acetate - MENOPUR”
“Other prescription drugs not on the Specialty Fertility List should still be obtained through participating retail or mail-service pharmacies under your prescription drug plan to obtain in-network benefits. - Specialty pharmaceuticals may be subject to medical necessity review.”
WINFertility for some employers
Horizon's site for Organon employees offers WINFertility, a family-building program with nurse care managers and behavioral health specialists around the clock. Ask your employer whether yours has a program like it.
“WINFertility, a family-building advocacy support program, will connect you with nurse care managers and behavioral health specialists, 24/7.”

Whether your Horizon Blue Cross Blue Shield plan covers IVF
The Horizon Blue Cross Blue Shield policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.
- 1.Your employer's choice
Most people with Horizon Blue Cross Blue Shield coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.
- 2.Your plan type
A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.
- 3.Your state's law
Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.

- 2Call Pharmacy Member Services at the number on the back of your Horizon card to check fertility drug coverage, and fill listed fertility drugs at a participating specialty pharmacy.
