On a fully insured large-group plan, it has to: California's SB 729 reached those plans at renewals from January 1, 2026, and Blue Shield's own guidance says those employers cannot opt out, except qualifying religious employers. The coverage includes up to three completed egg retrievals with unlimited embryo transfers. Small business plans exclude it by default, though a small business can choose a version of its plan that includes it, and self-funded employers may opt in. The optional fertility riders are not available on individual and family plans. Your final Evidence of Coverage and Summary of Benefits decide.

Who counts as infertile

““infertility” means a condition or status characterized by any of the following: 1. A licensed physician’s findings, based on a patient’s medical, sexual, and reproductive history, age, physical findings, diagnostic testing, or any combination of those factors.”

Pregnancy resulting in miscarriage does not restart the 12- month or 6-month time period to qualify as having infertility.”

Freezing eggs, sperm or embryos

The medical policy (effective April 1, 2026) says freezing embryos, eggs, ovarian tissue, sperm or testicular tissue for a person's infertility may be considered medically necessary, and it cites California's law on fertility preservation for fully insured plans when a treatment may cause infertility.

“II. Cryopreservation of embryos, oocytes, ovarian tissue, sperm, or testicular tissue (in post- pubertal men) for infertility of a person may be considered medically necessary.”

What decides it

Whether your Blue Shield of California plan covers IVF

The Blue Shield of California policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.

  1. 1.Your employer's choice

Most people with Blue Shield of California coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.

  2. 2.Your plan type

A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.

  3. 3.Your state's law

Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.

What specific employers publish about their fertility benefits, including which insurer or benefit company runs them: companies that cover IVF.

By state

Does Blue Shield of California cover IVF in California?

California's insurance law applied to a Blue Shield of California plan issued there. A state law reaches plans issued in that state, usually your employer's state, not where the clinic is.

Must cover IVF (1)
  • Blue Shield of California in California

Often, on fully-insured plans: California requires some fully-insured health plans to cover IVF, and that includes Blue Shield of California plans issued in California unless one of the law's limits or exemptions applies. New for 2026: SB 729 requires large-group fully-insured plans to cover IVF. A self-funded employer plan that Blue Shield of California only administers is not bound by state law and covers IVF only if the employer chose to. California law and clinics

How to see what your Blue Shield of California plan covers

A policy or program page is a summary. Your own plan's documents are what it pays by.

  1. 1Find out how you get your plan: a large employer (fully insured or self-funded), a small business, or on your own. Each has different fertility rules.
  2. 2Read your final Evidence of Coverage and Summary of Benefits: Blue Shield says they hold the current approved benefits.
  3. 3Look for Infertility Services in the Summary of Benefits for the limits (egg retrievals per lifetime) and your cost share.
  4. 4If your employer is self-funded, ask HR whether it opted in to the SB 729 fertility coverage.
  5. 5Before treatment, confirm the clinic is a Blue Shield participating provider and that Blue Shield has authorized the services.
Questions

Blue Shield of California and IVF: common questions

Does Blue Shield of California cover IVF?

On fully insured large-group plans, yes since renewals from January 1, 2026: California's SB 729 requires it and Blue Shield says those employers cannot opt out, except qualifying religious employers. Small business plans exclude it by default but can include it, self-funded employers may opt in, and the fertility riders are not available on individual and family plans.

How many IVF cycles does Blue Shield of California cover?

On large-group plans under SB 729, up to three completed egg retrievals with unlimited embryo transfers, using a single embryo transfer when recommended. One 2026 large-group plan summary lists egg retrieval limited to 3 per lifetime.

Does Blue Shield of California cover IVF on a small business plan?

Not by default: SB 729 coverage is excluded for small business groups unless the employer chooses a plan version that includes it. The separately sold small-business fertility rider does not cover IVF.

Does Blue Shield of California cover IVF for single people or same-sex couples?

California's definition of infertility, which Blue Shield's policy uses, includes a person's inability to reproduce as an individual or with their partner without medical intervention, so the definition does not depend on having a partner.

Does Blue Shield of California cover egg freezing?

Its medical policy, effective April 1, 2026, says freezing embryos, eggs, ovarian tissue or sperm for a person's infertility may be considered medically necessary, and it cites California's law on fertility preservation when a treatment may cause infertility. What your plan pays depends on its benefits.
