What Excellus BlueCross BlueShield publishes about covering IVF and other fertility treatment, in its own words, with the document each statement is in.

It depends on your contract. Excellus's assisted reproduction policy (effective April 15, 2026) says IVF and insemination are contract dependent. Its pharmacy policy applies New York's rule that large-group plans with comprehensive coverage cover three IVF cycles, a lifetime maximum per enrollment, while its sample PPO contract lists IVF among what it does not cover. Where a contract has the benefit but sets no limit, Excellus applies three IVF cycles and six inseminations per lifetime.
What Excellus' own documents say
Read on Oct 22, 2026. The words in quotation marks are the source's own.
Large-group plans: 3 IVF cycles
“All Assisted Reproductive Technologies (ART), including but not limited to, in vitro fertilization (IVF) and artificial insemination, are contract dependent.”
“Large group policies that provide medical, major medical or similar comprehensive type coverage shall provide coverage for three cycles of in-vitro fertilization used in the treatment of infertility.”
“Exclusions and Limitations. We do not Cover: · In vitro fertilization; · Gamete intrafallopian tube transfers or zygote intrafallopian tube transfers;”
How many cycles
On large-group plans, three IVF cycles, a lifetime maximum per enrollment with Excellus. Where a contract has the benefit but sets no limit, Excellus applies three IVF cycles and six artificial inseminations per lifetime. A cycle starts when drugs are given to prepare for IVF with embryo transfer.
“The three-cycle limit is a lifetime maximum of three cycles per enrollment with HealthPlan.”
“Artificial insemination is limited to a lifetime maximum of six (6) cycles; and B. IVF is limited to a lifetime maximum of three cycles.”
“A “cycle” is considered all treatment that starts when: preparatory medications are administered with the intent of undergoing IVF with embryo transfer.”
Inseminations first
IVF is indicated after six to 12 unsuccessful insemination cycles under 35, and fewer at 35 or older.
“For individuals younger than 35 years of age, when a successful pregnancy has not been achieved after six to 12 cycles of assisted insemination (e.g., artificial insemination or therapeutic donor insemination); OR C. For individuals 35 years or older, when a successful pregnancy has not been achieved after three to six cycles of assisted insemination”
Who qualifies
Excellus applies New York's definition of infertility, and its policy says the definition may not be used to deny or delay treatment because of relationship status or sexual orientation. Basic treatments such as IUI must be provided to people unable to conceive because of their sexual orientation or gender identity.
“New York Insurance Law §4303 defines infertility as “a disease or condition characterized by the incapacity to impregnate another person or to conceive, due to the failure to establish a clinical pregnancy after 12 months of regular, unprotected sexual intercourse or therapeutic donor insemination, or after six months of regular unprotected sexual intercourse or therapeutic donor insemination for a female 35 years of age or older.””
“Nothing in this definition shall be used to deny or delay treatment to any individual, regardless of relationship status or sexual orientation.”
“Basic infertility treatments (e.g., intrauterine insemination procedures) must be provided to individuals who are unable to conceive due to their sexual orientation or gender identity.”
Fertility drugs
Drugs used with IVF are covered for three cycles on large-group plans, or as your plan says, with prior authorization; Follistim needs a trial of Gonal-f first.
“Medications used in conjunction with in-vitro fertilization in the treatment of infertility will be covered: a. For three cycles for members covered under large group policies that provide medical, major medical or similar comprehensive type coverage.”
“when IVF limits have not been exceeded, are covered, subject to prior authorization requirements, regardless of whether the HealthPlan member has drug coverage under his/her policy.”
“Step therapy applies: All requests for Follistim AQ must have a trial and failure of Gonal-f first”
Fertility preservation
Standard fertility preservation is covered when a medical treatment may cause infertility, as New York law has required since 2020; freezing only to delay having children is not.
“In 2020, the law was expanded to require coverage of the following fertility preservation services under most insured managed care and indemnity health plans.”
“Standard fertility preservation services when a medical treatment is necessary to correct malformation, disease or dysfunction that may, directly or indirectly, cause iatrogenic infertility; or when fertility is impaired by surgery, radiation, chemotherapy or other medical treatment affecting reproductive organs or processes.”
“Under the statute, fertility preservation services solely for the purpose of delaying reproduction are not eligible for coverage.”
Not covered or not medically necessary
Embryo transfer to a gestational carrier, IVF cycles started only to bank embryos, assisted reproduction after a tubal ligation or vasectomy, and assisted hatching, among others. Embryo genetic testing is not by itself a reason for IVF.
“The implantation of eggs, donor sperm, or embryo(s) into a gestational carrier/surrogate/host uterus, regardless of their Health Plan member status is not covered.”
“IVF cycles initiated for the sole purpose of embryo banking are considered not medically necessary.”
“ART are not medically necessary for members who have undergone permanent birth control procedures (e.g. bilateral tubal ligation or vasectomy) as they do not meet the definition of infertility”
“A. Assisted embryo hatching; (CPT 89253) B. Hyaluronan binding assay (HBA); C. Co-culture of embryos; (CPT 89251)”
“Preimplantation genetic testing is not an indication for IVF.”
Whether your Excellus plan covers IVF
The Excellus policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.
- 1.Your employer's choice
Most people with Excellus coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.
- 2.Your plan type
A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.
- 3.Your state's law
Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.

Does Excellus cover IVF in New York?
New York's insurance law applied to a Excellus plan issued there. A state law reaches plans issued in that state, usually your employer's state, not where the clinic is.
Excellus in New York
Often, on fully-insured plans: New York requires some fully-insured health plans to cover IVF, and that includes Excellus plans issued in New York unless one of the law's limits or exemptions applies. Large-group fully-insured plans must cover IVF (up to 3 cycles). A self-funded employer plan that Excellus only administers is not bound by state law and covers IVF only if the employer chose to. How to see what your Excellus plan covers
A policy or program page is a summary. Your own plan's documents are what it pays by.
- 1Read your subscriber contract's infertility section: Excellus says IVF is contract dependent.
- 2Ask HR whether your plan is a fully insured large-group plan: New York requires those to cover three IVF cycles, and a self-funded plan covers what the employer chose.
- 3Ask your clinic to request prior authorization for IVF drugs before your cycle starts.
- 4Call the member service number on your Excellus card to confirm your limits.
