Does Florida Blue cover IVF?
What Florida Blue (Blue Cross and Blue Shield of Florida) publishes about covering IVF and other fertility treatment, in its own words, with the document each statement is in.

It depends on your benefits. Florida Blue's infertility guideline (approved May 28, 2026) says infertility treatment, IVF and insemination vary by the member's benefits and are subject to the contract's limits. Where a plan has the benefit and infertility is documented for a year, it treats four cycles of IVF or insemination as medically necessary and reviews more case by case. A large-group BlueOptions sample booklet excludes IVF, and Florida's benchmark plan, a Florida Blue small-group plan, does not cover infertility treatment.
What Florida Blue's own documents say
Read on Oct 22, 2026. The words in quotation marks are the source's own.
Depends on your benefits
“Infertility diagnosis, treatment, assisted reproductive technologies (e.g., IVF, ZIFT, GIFT) and artificial and intrauterine insemination (AI) vary by member’s benefits.”
“NOTE: Coverage for infertility is subject to the member’s benefit terms, limitations and maximums. Refer to contract language regarding infertility.”
Four cycles where the benefit exists
If the plan has the benefit and infertility is documented for at least a year, four cycles of IVF, ZIFT, GIFT or insemination meet the guideline; more than four are reviewed for medical appropriateness.
“If the benefit for coverage for infertility treatment is available and there is documentation of infertility for at least one year’s duration, the following indications meet the definition of medical necessity:”
“Four (4) cycles for any Assisted Reproductive Technologies (ARTs): – IVF, ZIFT, GIFT, AI”
“If medical circumstances warrant consideration of more than four (4) cycles, additional cycles will be reviewed for medical appropriateness”
How Florida Blue defines infertility
Unable to conceive after trying for a year, or unable to carry a pregnancy to live birth.
“Infertility is defined as inability to conceive a child despite trying for one year, or the inability of the woman to carry a pregnancy that results in live birth.”
Fertility drugs
Depend on your contract benefits.
“For fertility drug coverage, refer to member’s contract benefits.”
Egg and ovarian tissue freezing
Freezing, storing and thawing eggs or ovarian tissue is considered experimental or investigational.
“Cryopreservation of oocyte(s), including storage and thawing of oocytes, is considered experimental or investigational.”
“Cryopreservation of ovarian tissue, including storage and thawing of ovarian tissue is considered experimental or investigational.”
Not eligible or experimental
Family balancing (choosing a baby's sex for non-medical reasons) is not eligible, and ICSI without male-factor infertility is considered experimental.
“Family balancing is not eligible for coverage.”
“Intracytoplasmic sperm injection in the absence of male factor infertility is considered experimental or investigational.”
Federal and state employees
Federal Employee Program and State of Florida plan members follow their own program's rules.
“Federal Employee Program (FEP): Follow FEP guidelines. State Account Organization (SAO): Follow SAO guidelines.”

Whether your Florida Blue plan covers IVF
The Florida Blue policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.
- 1.Your employer's choice
Most people with Florida Blue coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.
- 2.Your plan type
A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.
- 3.Your state's law
Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.
What specific employers publish about their fertility benefits, including which insurer or benefit company runs them: companies that cover IVF.
Does Florida Blue cover IVF in Florida?
Florida's insurance law applied to a Florida Blue plan issued there. A state law reaches plans issued in that state, usually your employer's state, not where the clinic is.

Not because of state law: Florida has no law requiring private plans to cover IVF, so IVF under a Florida Blue plan there depends entirely on what the employer chose (or, for a plan you buy yourself, on the plan's own terms). Florida law and clinics

How to see what your Florida Blue plan covers
A policy or program page is a summary. Your own plan's documents are what it pays by.
- 1Read your benefit booklet's What Is Not Covered section and its infertility benefit: the guideline says coverage varies by your benefits.
- 2If your plan has the benefit, keep records showing at least a year of infertility: the guideline's indications require it.
- 3Check your contract's pharmacy benefits for fertility drugs.
- 4Call the member service number on your Florida Blue card to confirm before treatment.
