What PacificSource Health Plans (Oregon, Idaho, Montana, Washington) publishes about covering IVF and other fertility treatment, in its own words, with the document each statement is in.

Not on its Idaho plans. PacificSource's list of services its Idaho medical plans do not cover names artificial insemination, IVF and treating infertility, with the tests, surgery and drugs to diagnose or treat it. For Oregon, Washington and Montana it publishes nothing that names IVF: it says each plan's exclusions are in its member handbook or policy, and only the policy's own language is binding.

Not on its Idaho plans; elsewhere your handbook decides
“Below is a complete list of services and treatments that are not covered under our medical plans.”
“Infertility – Services and supplies for artificial insemination, in vitro fertilization, treatment of Infertility, erectile dysfunction, sexual dysfunction, or surgery to reverse voluntary sterilization.”
Everything around infertility, in Idaho
The Idaho list also excludes the lab work, imaging, surgery, treatment and prescription drugs to diagnose, prevent or cure infertility or induce fertility, GIFT and ZIFT included.
“Services and supplies, diagnostic laboratory and x-ray studies, surgery, treatment, or Prescription Drugs to diagnose, prevent, or cure Infertility or to induce fertility (including Gamete and/ or Zygote Intrafallopian Transfer; such as GIFT or ZIFT).”
Your handbook decides
PacificSource says a plan may exclude some procedures and medications, that the member handbook or policy has your plan's details, and that only the policy's language is legally binding. Oregon plans follow the state's reproductive-health coverage rules, which it does not spell out for infertility.
“Your plan may exclude some procedures, services, and medications. You'll find information specific to your plan in your member handbook or policy.”
“A full explanation of benefits, including limitations and exclusions, will be provided in your policy. Only the language of the actual policy is legally binding.”
“The state of Oregon requires all health benefit plans to cover certain services, drugs, devices, products, and procedures relating to reproductive health and functioning. All PacificSource plans comply with these rules, which you can read at the OregonLaws.org website.”
Whether your PacificSource plan covers IVF
The PacificSource policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.
- 1.Your employer's choice
Most people with PacificSource coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.
- 2.Your plan type
A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.
- 3.Your state's law
Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.
What specific employers publish about their fertility benefits, including which insurer or benefit company runs them: companies that cover IVF.
Does PacificSource cover IVF in your state?
Each state's insurance law applied to a PacificSource plan issued there. A state law reaches plans issued in that state, usually your employer's state, not where the clinic is.
PacificSource in Montana
Not because of state law: Montana requires some infertility coverage but not IVF, so IVF under a PacificSource plan there depends on what the employer chose. Montana law and clinics
PacificSource in Idaho
Not because of state law: Idaho has no law requiring private plans to cover IVF, so IVF under a PacificSource plan there depends entirely on what the employer chose (or, for a plan you buy yourself, on the plan's own terms). Idaho law and clinics
PacificSource in Oregon
Not because of state law: Oregon has no law requiring private plans to cover IVF, so IVF under a PacificSource plan there depends entirely on what the employer chose (or, for a plan you buy yourself, on the plan's own terms). Oregon law and clinics
PacificSource in Washington
Not because of state law: Washington has no law requiring private plans to cover IVF, so IVF under a PacificSource plan there depends entirely on what the employer chose (or, for a plan you buy yourself, on the plan's own terms). Washington law and clinics

How to see what your PacificSource plan covers

- 2Look for infertility in your handbook's exclusions: the Idaho list excludes IVF and infertility treatment.
- 3On a plan through work, ask HR whether the employer added a fertility benefit.
