Oregon's state employee (PEBB) Moda plans list IVF as covered, with fertility benefits through Progyny from 2027. Your own plan document decides.

What decides it

Whether your Moda Health plan covers IVF

The Moda Health policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.

  1. 1.Your employer's choice

Most people with Moda Health coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.

  2. 2.Your plan type

A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.

  3. 3.Your state's law

Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.

What specific employers publish about their fertility benefits, including which insurer or benefit company runs them: companies that cover IVF.

By state

Does Moda Health cover IVF in your state?

Each state's insurance law applied to a Moda Health plan issued there. A state law reaches plans issued in that state, usually your employer's state, not where the clinic is.

No state law (2)
  • Moda Health in Alaska

Not because of state law: Alaska has no law requiring private plans to cover IVF, so IVF under a Moda Health plan there depends entirely on what the employer chose (or, for a plan you buy yourself, on the plan's own terms). Alaska law and clinics

  • Moda Health in Oregon

Not because of state law: Oregon has no law requiring private plans to cover IVF, so IVF under a Moda Health plan there depends entirely on what the employer chose (or, for a plan you buy yourself, on the plan's own terms). Oregon law and clinics

How to see what your Moda Health plan covers

A policy or program page is a summary. Your own plan's documents are what it pays by.

  1. 1Read your Moda member handbook or policy for infertility exclusions: the standard plans exclude treatment.
  2. 2If you are an Oregon public employee, check your board's benefits: PEBB plans cover IVF, the OEBB Moda plan does not.
  3. 3Ask your employer whether it added a fertility benefit to its Moda plan.
Questions

Moda Health and IVF: common questions

Does Moda Health cover IVF?

Does the Oregon educators' Moda plan cover IVF?

No. The 2025-26 OEBB Moda handbook covers infertility diagnosis, ovulation induction and IUI, but not IVF or other advanced reproductive services.

Does Oregon require insurance to cover IVF?

No Oregon law requires private plans to cover IVF, so a Moda plan covers it only if the plan includes it.
