Does Blue Cross Vermont cover IVF?
What Blue Cross and Blue Shield of Vermont (Blue Cross VT) publishes about covering IVF and other fertility treatment, in its own words, with the document each statement is in.

It depends on the plan. Blue Cross VT's 2026 certificate for its individual Platinum, Gold, Silver and Bronze plans lists infertility services, insemination and IVF included, as a general exclusion, and excludes fertility drugs; testing to find out whether and why a couple is infertile is covered. Its infertility policy for self-funded employer groups lists IVF, up to four cycles, as eligible once its criteria are met, within a $15,000 lifetime maximum, where the employer's plan includes it.
What Blue Cross VT's documents say
Read on Oct 26, 2026. The words in quotation marks are the source's own.
Not on its individual plans; some self-funded employers cover 4 cycles
“In addition to the specific exclusions listed elsewhere in this Contract, the following General Exclusions apply:”
“surgical, radiological, pathological, or laboratory procedures leading to or in connection with (for example): ʂ insemination (intravaginal, intracervical, and intrauterine insemination); ʂ in vitro fertilization, embryo transplantation and gamete intrafallopian transfer (GIFT);”
“Coverage varies according to the member’s group or individual contract. Not all groups are required to follow the Vermont legislative mandates. Member Contract language takes precedence over medical policy when there is a conflict.”
Testing is covered
The certificate covers the evaluation and testing to find out whether and why a couple is infertile; the exclusion does not apply to it.
“We also cover the evaluation and diagnostic testing to determine if and why a couple is infertile.”
“Note: This exclusion does not apply to the evaluation to determine if and why a couple is infertile.”
Fertility drugs
The certificate's drug benefit excludes all medications for treating infertility, naming Clomid, Gonal-F, Follistim, Novarel, Ovidrel and Pregnyl, and the 2026 plan summary lists infertility medications among services the plan does not cover.
“We do not provide Prescription Drugs and Biologics benefits for:”
“all medications for treatment of infertility, including but not limited to Clomid, Clomiphene, Serophene, Bravelle, Gonal-F, Follistim AQ, Novarel, Ovidrel, Pregnyl, Profasi and Repronex when used for treatment of infertility;”

Vermont law and your group
Blue Cross VT says coverage varies by group or individual contract, that not all groups must follow Vermont's legislative mandates, and that your contract wins over the policy.
“Coverage varies according to the member’s group or individual contract. Not all groups are required to follow the Vermont legislative mandates. Member Contract language takes precedence over medical policy when there is a conflict.”
Prior approval
Infertility treatment is on Blue Cross VT's prior approval list where benefits apply.

Whether your Blue Cross Vermont plan covers IVF
The Blue Cross Vermont policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.
- 1.Your employer's choice
Most people with Blue Cross Vermont coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.
- 2.Your plan type
A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.
- 3.Your state's law
Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.
What specific employers publish about their fertility benefits, including which insurer or benefit company runs them: companies that cover IVF.
Does Blue Cross Vermont cover IVF in Vermont?
Vermont's insurance law applied to a Blue Cross Vermont plan issued there. A state law reaches plans issued in that state, usually your employer's state, not where the clinic is.
Blue Cross Vermont in Vermont
Not because of state law: Vermont has no law requiring private plans to cover IVF, so IVF under a Blue Cross Vermont plan there depends entirely on what the employer chose (or, for a plan you buy yourself, on the plan's own terms). Vermont law and clinics

How to see what your Blue Cross Vermont plan covers
A policy or program page is a summary. Your own plan's documents are what it pays by.
- 1On an individual plan, read your certificate's General Exclusions: infertility treatment is excluded, testing is covered.
- 2On a plan through work, ask HR whether the plan is self-funded and includes infertility benefits, then read the employer's plan documents.
- 3Call Blue Cross VT customer service to confirm your benefits, and get prior approval before treatment.
