Does Blue Cross of Idaho cover IVF?
What Blue Cross of Idaho publishes about covering IVF and other fertility treatment, in its own words, with the document each statement is in.

No, on its standard plans. Blue Cross of Idaho's 2027 plan guides for individual, small group and medium and large group plans list treatment for reproductive procedures, including ovulation induction, intrauterine insemination, IVF and embryo transfer, among the general exclusions, and exclude drugs prescribed mainly to increase fertility. A self-funded group's own plan can differ: its plan document decides.

“GENERAL EXCLUSIONS AND LIMITATIONS”

“Treatment for reproductive procedures, including but not limited to, ovulation induction procedures and pharmaceuticals, intrauterine insemination, in vitro fertilization, embryo transfer or similar procedures, or procedures that in any way augment or enhance an Insured's reproductive ability, including but not limited to laboratory services, radiology services or similar services related to treatment for reproduction procedures.”
Individual and group plans alike
The same exclusion appears in the individual, small group and medium and large group guides for 2027, and in the 2026 individual guide.
“Treatment for reproductive procedures, including but not limited to, ovulation induction procedures and pharmaceuticals, intrauterine insemination, in vitro fertilization, embryo transfer or similar procedures, or procedures that in any way augment or enhance a Member’s reproductive ability, including but not limited to laboratory services, radiology services or similar services related to treatment for reproduction procedures.”

Fertility drugs
Drugs prescribed primarily to increase fertility, including those that induce or enhance ovulation, are not covered under the prescription drug benefit.
“apply to Prescription Drug Services. No benefits are available under this Contract for the following:”
“Prescribed primarily to increase fertility, including but not limited to, drugs which induce or enhance ovulation.”
Freezing before cancer treatment
From November 1, 2026, its medical policy treats freezing mature eggs and sperm as medically necessary before chemotherapy, radiation or removal of both ovaries, not for normal reproductive aging; some plans exclude any freezing or storage charges, so check yours.
“Cryopreservation of mature oocytes and sperm is considered medically necessary when all the criteria is met:”
“Anticipated infertility due to medical or surgical treatment such as gonadotoxic therapies (i.e., chemotherapy, radiation therapy) or bilateral oophorectomy; AND”
“The anticipated infertility is not the result of normal reproductive aging or sterilization services (when not for treatment of an illness, injury or disease).”
“Some plans have a specific contractual exclusion of coverage of any charges associated with cryopreservation or storage. Please check benefit plan descriptions.”
Self-funded groups
Coverage varies by your member-specific plan document, which overrides the policy, and the policy may not apply to certain self-insured groups.
“Coverage of infertility services varies by the member-specific benefit plan document and may be additionally governed by state mandates.”
“This policy may not apply to all lines of business such as the Federal Employee Program, Medicare Supplement, Medicare Advantage, Medicaid, and certain self-insured groups.”
Also excluded
Reversing a sterilization.
“For the reversal of sterilization procedures, including but not limited to, vasovasostomies or salpingoplasties.”
Whether your Blue Cross of Idaho plan covers IVF
The Blue Cross of Idaho policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.
- 1.Your employer's choice
Most people with Blue Cross of Idaho coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.
- 2.Your plan type
A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.
- 3.Your state's law
Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.
What specific employers publish about their fertility benefits, including which insurer or benefit company runs them: companies that cover IVF.
Does Blue Cross of Idaho cover IVF in Idaho?
Idaho's insurance law applied to a Blue Cross of Idaho plan issued there. A state law reaches plans issued in that state, usually your employer's state, not where the clinic is.
Blue Cross of Idaho in Idaho
Not because of state law: Idaho has no law requiring private plans to cover IVF, so IVF under a Blue Cross of Idaho plan there depends entirely on what the employer chose (or, for a plan you buy yourself, on the plan's own terms). Idaho law and clinics
