No, not on the plans we read. BCBSMT's 2026 contracts and member guides for individual, small group, mid-market and self-funded Blue Balance Funded plans all say the plan will not pay for services, supplies, drugs or devices related to IVF, or for drugs used to treat infertility. The same plans pay for tests to find the cause of infertility, artificial insemination or IUI, and fertility preservation when a medical treatment may cause infertility. Some self-funded employer and governmental plans it administers set their own rules.

Your plan document governs
BCBSMT says benefit plans vary, that a plan document wins over a medical policy, and that some self-funded employer or governmental plans it administers may not use its medical policy.
“Benefit plans vary in coverage and some plans may not provide coverage for certain services discussed in the Medical Policies.”
“In the event of conflict between a Medical Policy and any plan document under which a member is entitled to Covered Services, the plan document will govern.”
“some benefit plans administered by BCBSMT, such as some self-funded employer plans or governmental plans, may not utilize BCBSMT Medical Policy”

Whether your Blue Cross Blue Shield of Montana plan covers IVF
The Blue Cross Blue Shield of Montana policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.
- 1.Your employer's choice
Most people with Blue Cross Blue Shield of Montana coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.
- 2.Your plan type
A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.
- 3.Your state's law
Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.
