One IVF procedure, once. HMSA's 2026 individual plan guides cover a one-time benefit for one outpatient IVF procedure, counted over all your time with HMSA in a group or individual plan, as Hawaii law requires of plans with pregnancy benefits. It needs precertification and, with a male partner, a five-year history of infertility or a listed condition; without one, three failed IUI cycles. Freezing eggs, sperm or embryos, donor eggs and sperm, and IVF with a surrogate are not covered.

Hawaii's IVF law
Hawaii requires individual and group plan contracts that provide pregnancy-related benefits to include a one-time benefit for outpatient IVF. The statute's own wording requires the patient's eggs to be fertilized with the spouse's sperm; HMSA's 2026 guides also set criteria for members without a male partner.
“All individual and group hospital or medical service plan contracts which provide pregnancy-related benefits shall include in addition to any other benefits for treating infertility, a one-time only benefit for all outpatient expenses arising from in vitro fertilization procedures”
“The patient's oocytes are fertilized with the patient's spouse's sperm;”
Whether your HMSA plan covers IVF
The HMSA policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.
- 1.Your employer's choice
Most people with HMSA coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.
- 2.Your plan type
A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.
- 3.Your state's law
Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.
What specific employers publish about their fertility benefits, including which insurer or benefit company runs them: companies that cover IVF.
Does HMSA cover IVF in Hawaii?
Hawaii's insurance law applied to a HMSA plan issued there. A state law reaches plans issued in that state, usually your employer's state, not where the clinic is.
HMSA in Hawaii
Often, on fully-insured plans: Hawaii requires some fully-insured health plans to cover IVF, and that includes HMSA plans issued in Hawaii unless one of the law's limits or exemptions applies. Fully-insured plans must cover a round of IVF under set conditions. A self-funded employer plan that HMSA only administers is not bound by state law and covers IVF only if the employer chose to. How to see what your HMSA plan covers
A policy or program page is a summary. Your own plan's documents are what it pays by.

- 2Find your Guide to Benefits in My Account, or ask your employment office or HMSA Customer Relations if it is not online.
- 3Get precertification before the IVF procedure, and check whether you have already used the one-time benefit with HMSA.
