No, not on the plans we read. BCBSOK's 2026 individual Gold plan and a 2026 employer booklet both exclude IVF, artificial insemination, ovulation induction and embryo transfer, and infertility drugs, except standard fertility preservation when a treatment may cause infertility. The employer booklet does cover diagnosing infertility and treating its underlying cause. Its medical policy says member contracts may vary, so your own contract decides.

Individual and family plans
Among its exclusions, the 2026 Blue Advantage Gold outline of coverage lists diagnosis, treatment and drugs for infertility and fertilization procedures, naming artificial insemination, ovulation induction, IVF and embryo transfer, except care for infertility a medical treatment causes. The 2026 MyBlue Bronze summary lists infertility treatment as generally not covered.

Employer plans
A 2026 Blue Balance Funded employer booklet lists IVF, artificial insemination, ovulation induction and embryo transfer among services that are not covered, and excludes infertility and fertility medications from the pharmacy benefit. It covers diagnosing infertility and treating its underlying cause, up to the limit in the plan's summary of benefits.
“The following are not covered services under your plan.”
“Any services or supplies provided for, in preparation for, or in conjunction with any of the following, except standard fertility preservation services to treat iatrogenic infertility:”
“In vitro fertilization”
“Infertility and fertility medications.”
“Diagnosis of infertility and treatment of the underlying cause. Infertility Treatment services are limited to the amount specified in the SUMMARY OF BENEFITS.”
Fertility preservation
Standard fertility preservation is covered for people of reproductive age when a medically necessary treatment may cause infertility, without prior authorization, but storage is not included. Oklahoma's SB 1334 requires it for people diagnosed with cancer on plans issued or renewed from January 1, 2025.
“Benefits will be provided for those who are within reproductive age, when medically necessary treatments may directly or indirectly cause iatrogenic infertility.”
“Standard fertility preservation services are not subject to prior authorization requirements.”
“standard fertility preservation services shall not include storage.”
“For Oklahoma only: For policies (Fully Insured PPO/HMO, Small Group, Mid-Market, Large Group, Municipalities/Schools, State Employee Plan) offered, issued, or renewed on or after January 1, 2025, SB 1334 (Section 6060.8b of Title 36) requires coverage for standard fertility preservation services, only for individuals diagnosed with cancer”
Your contract decides
BCBSOK's infertility policy tells members and providers to check the benefit plan, summary plan description or contract, and says member contract benefits may vary.
“Member contract benefits may vary. CHECK CONTRACTS FOR COVERAGE ELIGIBILITY of services related to reproductive technologies/techniques”
“Members and their providers have the responsibility for consulting the member's benefit plan, summary plan description or contract to determine if there are any exclusions or other benefit limitations applicable to this service or supply.”
Whether your Blue Cross Blue Shield of Oklahoma plan covers IVF
The Blue Cross Blue Shield of Oklahoma policy says how it pays for IVF when a plan includes it. Three things decide whether yours does.
- 1.Your employer's choice
Most people with Blue Cross Blue Shield of Oklahoma coverage get it through work, and the employer picks the benefits. About 50% of employers with 500 or more workers now cover IVF.
- 2.Your plan type
A fully-insured plan follows the law of the state it is issued in. A self-funded plan, which 67% of covered US workers have, does not: it covers whatever the employer chose.
- 3.Your state's law
Some states require fully-insured plans to cover IVF, some require insurers only to offer it, and most require nothing. Your state's line is below.
