IVF Medication Costs After TrumpRx: Every Posted Price, Compared
2026-10-04 · 14 min read · IVFcost.co
What the medications for one IVF cycle cost in 2026, drug by drug, depending on where you fill the prescription.
The same four IVF drugs cost $3,776.50 for a standard cycle at the lowest cash prices anyone can get, and $16,505.41 at an ordinary retail pharmacy's cash prices: more than four times as much. We read every cash price posted for IVF medications on October 4, 2026: the government's TrumpRx.gov, the manufacturers' own programs, US fertility and cash pharmacies, and the Drugs.com retail price guide. We priced the "standard cycle" the White House's own economists use: 10 days of Gonal-f at 300 IU and Menopur at 150 IU, 5 days of Cetrotide and one Ovidrel trigger shot ( Council of Economic Advisers, May 2026, footnote 20). Your own protocol will differ, because doses are set to how your body responds, but the comparison holds drug by drug.
The biggest change of the year is TrumpRx, the federal cash-price site, which since early 2026 carries EMD Serono's discounted prices for Gonal-f, Cetrotide and Ovidrel. Every price below links to the page we read it on, and each medication has its own page with every posted price: all IVF medication prices.
Key findings
- A standard cycle's drugs cost $3,776.50 at the lowest prices open to anyone: Gonal-f at the TrumpRx price ($1,680), Menopur at the one fertility pharmacy that posts a price ($1,900), Cetrotide ($112.50) and Ovidrel ($84). At retail cash prices the same drugs cost $16,505.41.
- TrumpRx cut EMD Serono's three IVF drugs by 67% to 93% off list price: Gonal-f to $42 per 75 IU (list $241.51), Cetrotide to $22.50 a dose (list $316.12) and Ovidrel to $84 (list $251.84). Cash only, not with insurance.
- Menopur is now the largest drug cost in the cycle. It is not on TrumpRx, and at $95 a vial its 20 vials ($1,900) cost more than the 3,000 IU of Gonal-f ($1,680). Its maker, Ferring, sells it for $72.75 a vial only for elective egg freezing paid in cash.
- The White House's "$2,996 per cycle" uses the income-qualified Gonal-f price of $25 per 75 IU. At the $42 TrumpRx price anyone paying cash gets, the same calculation comes to $3,676.50, a saving of $1,510.50 rather than $2,191.
- The TrumpRx coupon works at only two pharmacies, Freedom Fertility and Village Fertility, by mail. EMD Serono's own savings card gives the same prices at eight.
- Only 1 of the 12 US fertility pharmacies we checked posts its prices (Mandell's Clinical Pharmacy). The rest quote on request, so the price you are first quoted is worth comparing.
- A generic is not always cheaper now. Generic cetrorelix costs $59.88 a kit at Cost Plus Drugs, plus $55 refrigerated shipping, against $22.50 for brand Cetrotide at EMD Serono's cash price.
How we checked
On October 4, 2026 we read, on each source's own current page:
- TrumpRx.gov, every fertility drug it lists. Only Gonal-f, Cetrotide and Ovidrel carry a fixed TrumpRx price; Menopur, Follistim, Novarel, Pregnyl, Endometrin, Crinone and leuprolide are not on the site.
- The manufacturers' programs: EMD Serono (Gonal-f, Cetrotide, Ovidrel), Ferring (Menopur, Novarel, Endometrin), Organon (Follistim, ganirelix, Pregnyl) and AbbVie (Crinone).
- 12 US fertility pharmacies, for a published price list: Mandell's, Freedom Fertility, Village Fertility, MDR, FuzeRx (formerly Alto), Metro Drugs, Prima, SMP, APRI, CVS Specialty, Optum Specialty (formerly Avella) and Schraft's 2.0. Only Mandell's posts its own prices; Metro Drugs posts Ferring's egg freezing price and nothing else.
- Cash pharmacies that post prices: Mark Cuban Cost Plus Drugs and Amazon Pharmacy, for the generics and tablets.
- The Drugs.com price guide, for what an ordinary retail pharmacy charges a cash customer ("Quoted prices are for cash-paying customers and are not valid with insurance plans").
- CMS NADAC, the national average price US retail pharmacies pay to buy each drug, for context.
We compare prices per one unit (per 75 IU of FSH, per dose, per vial), because sellers list different package sizes. A price counts only if the seller states it on its own page; we excluded pharmacies outside the US. Prices change, and Mandell's calls its figures estimates and adds a service fee at checkout.
What a standard cycle's drugs cost, by where you buy them
The White House's economists define a standard IVF stimulation as 3,000 IU of Gonal-f and 1,500 IU of Menopur (20 vials) over 10 days, 5 doses of Cetrotide and one Ovidrel trigger. Priced at each kind of source:
| Drug (standard cycle amount) | Lowest price open to anyone | Retail cash price (Drugs.com) |
|---|---|---|
| Gonal-f, 3,000 IU | $1,680 ($42 per 75 IU, TrumpRx) | $9,507.19 (three 900 IU pens at $2,838.78 and one 300 IU pen at $990.85) |
| Menopur, 20 vials of 75 IU | $1,900 ($95 a vial, Mandell's) | $5,160.92 (four boxes of 5 at $1,290.23) |
| Cetrotide, 5 doses of 0.25 mg | $112.50 ($22.50 a dose, TrumpRx) | $1,583.40 ($316.68 a dose) |
| Ovidrel, one 250 mcg syringe | $84 ( TrumpRx) | $253.90 |
| Total | $3,776.50 | $16,505.41 |
Most of the gap is where you fill the prescription, not which drugs you take. A retail pharmacy's cash price for Gonal-f is about $237 per 75 IU; at TrumpRx and at the pharmacies honoring EMD Serono's card it is $42. Most IVF prescriptions are filled by fertility pharmacies that ship them to your door, many of them refrigerated.
For an elective egg freezing cycle with the same drugs, Ferring's HeartTomorrow price for Menopur ($72.75 a vial for at least 20 vials) brings the total to $3,331.50.
What TrumpRx changed
TrumpRx carries the discount EMD Serono agreed with the White House in October 2025. The "original price" TrumpRx shows beside each price is the list price (the wholesale acquisition cost):
| Medication | List price | TrumpRx price | Off list |
|---|---|---|---|
| Gonal-f, 300 IU pen | $966.04 | $168 ($42 per 75 IU) | 83% |
| Gonal-f, 450 IU pen or multi-dose vial | $1,449.06 | $252 | 83% |
| Gonal-f, 900 IU pen | $2,898.12 | $504 | 83% |
| Cetrotide, 0.25 mg | $316.12 | $22.50 | 93% |
| Ovidrel, 250 mcg syringe | $251.84 | $84 | 67% |
Who can use it. The price is for people paying cash: "This is an out-of-pocket price", and the coupon's terms exclude anyone seeking reimbursement from private or government insurance for these drugs. Insurance can still pay for the rest of your treatment.
Where to fill it. The TrumpRx coupon itself says it "ONLY works at Freedom Fertility and Village Fertility pharmacies", both by mail order. EMD Serono's own Fertility Instant Savings card, which its FAQ says gives "the same great discounts", is accepted at eight pharmacies: Freedom Fertility, Village Fertility, Alto (now FuzeRx), Mandell's, Apri, Prima, CVS Specialty and SMP Pharmacy Solutions. Mandell's posts the same $168, $252, $504, $22.50 and $84 on its own price list.
What it does not cover. TrumpRx has no page for Menopur, Follistim, Novarel, Pregnyl, Endometrin, Crinone or the leuprolide (Lupron) kit. For generics such as ganirelix and letrozole it shows pharmacy coupon prices that change with your ZIP code, not a fixed TrumpRx price.
Menopur, the drug TrumpRx left out
Menopur supplies the LH activity many protocols add to FSH, and it is now the single largest drug cost in the standard cycle:
- $95 a vial at Mandell's Clinical Pharmacy, the only fertility pharmacy we found that posts a Menopur price ( Mandell's).
- $72.75 a vial through Ferring's HeartTomorrow program, only for elective egg freezing cycles paid in cash, with a first fill of at least 20 vials; the offer ends December 31, 2026 ( Ferring).
- $258.05 a vial at an ordinary retail pharmacy ( Drugs.com).
- $252.12 a vial is what US retail pharmacies pay on average to buy it (CMS NADAC, effective September 23, 2026), so the fertility pharmacy's cash price is well below the retail pharmacy's own cost.
Ferring's HEART program gives rebates to patients with no fertility drug coverage at its in-network pharmacies, but Ferring publishes no amount ( Menopur). The White House estimate keeps Menopur at $1,800 for the standard cycle before and after the deal, so the TrumpRx savings it counts come entirely from EMD Serono's three drugs.
Every IVF drug, compared
The lowest cash price we found posted for each medication, per one unit, beside the retail cash price. Each drug's page lists every price with its seller and conditions.
| Medication | Per | Lowest posted cash price | Retail (Drugs.com) |
|---|---|---|---|
| Gonal-f | 75 IU | $42 (TrumpRx, Mandell's) | $236.57 |
| Follistim AQ | 75 IU | none posted | $213.11 |
| Menopur | 75 IU vial | $95 (Mandell's) | $258.05 |
| Cetrotide | 0.25 mg dose | $22.50 (TrumpRx, Mandell's) | $316.68 |
| Ganirelix | 250 mcg syringe | $45.25 (Cost Plus Drugs, generic) | from $37.38 |
| Lupron (leuprolide) kit | 14-day kit | $64.02 (Cost Plus Drugs, generic) | from $133.80 |
| Ovidrel | 250 mcg syringe | $84 (TrumpRx, Mandell's) | $253.90 |
| hCG (Novarel, Pregnyl, generic) | 10,000 units | $149.90 (Mandell's, generic) | $182.10 (Pregnyl) |
| Progesterone in oil | 10 mL vial | none posted | $22.25 |
| Endometrin | 100 mg insert | none posted | $15.60 |
| Clomid | 50 mg tablet | $4.08 (Cost Plus Drugs) | $4.50 |
| Letrozole | 2.5 mg tablet | $0.25 (Cost Plus Drugs) | $0.40 |
Two things stand out. Organon publishes no cash price for Follistim, its ganirelix or Pregnyl: its ReUnite Rx program gives "reduced, everyday pricing" at network pharmacies, but "to get the exact pricing for your fertility medication, you will need to contact the pharmacy" ( ReUnite Rx). And the cheap generics are real: a 14-day leuprolide kit is $64.02 at Cost Plus Drugs against $197.61 that retail pharmacies pay on average (NADAC), and generic ganirelix is $45.25 a syringe.
The White House figure assumes the income-qualified price
The Council of Economic Advisers wrote in May 2026 that "the cost of prescription drugs for a standard IVF cycle has decreased from $5,187 to $2,996, generating $2,191 in savings per IVF cycle." Its own footnote builds the $2,996 from "$1800 for Menopur, $1000 for Gonal-F, $112.5 for Cetrotide, and $84 for Ovidrel" ( CEA, page 6).
$1,000 for 3,000 IU of Gonal-f is $25 per 75 IU, the price the report gives for "families with income <550% of federal poverty level ... through a savings program enabled by EMD-Serono". The same paragraph says that above that income, "Gonal-F is offered at $42 per 75 units at TrumpRx.gov." EMD Serono's income-based program, Compassionate Care, asks for an application with a tax return and does not publish its price ( EMD Serono).
| Gonal-f price used | Standard cycle drug cost | Saving vs $5,187 |
|---|---|---|
| $25 per 75 IU (income-qualified, the CEA figure) | $2,996 | $2,191 |
| $42 per 75 IU (TrumpRx, anyone paying cash) | $3,676.50 | $1,510.50 |
Both rows use the CEA's own $90-a-vial Menopur. The report says families under that income line are 70% of the US population, so many patients may qualify for the lower price, but only after applying; the $42 price needs no application.

None of these programs pays referral fees, and IVFcost.co earns nothing from any of them.
How to pay less for IVF medications
- Check your pharmacy benefit first. A plan with no IVF coverage can still cover fertility drugs, and the cash programs above cannot be combined with insurance for the same drugs. Only 32% of employer plans cover fertility medications ( CEA).
- If you pay cash for Gonal-f, Cetrotide or Ovidrel, use the TrumpRx price through TrumpRx or EMD Serono's Fertility Instant Savings card, at a pharmacy that accepts it. If your household income may qualify, apply to Compassionate Care for a lower price.
- Get two or three quotes for everything else. Only one fertility pharmacy posts its prices, and one we checked (MDR) offers to beat a competitor's quote by $100. Menopur, Follistim and the hCG trigger are where quotes differ most.
- Ask about generics, and about shipping. Generic ganirelix and leuprolide are cheap at cash pharmacies, but refrigerated shipping can erase the saving on a single kit.
- Look at the program list if you are freezing eggs electively, have a cancer diagnosis, or are a veteran: several programs give medication free or at a fixed low price.
Medications are about a fifth of what an IVF cycle costs in total; the rest is the clinic's fees. Our IVF cost breakdown puts the drugs beside everything else, the egg freezing price census shows what clinics charge for the procedure, and paying for IVF and our IVF grants finder cover the rest of the bill.
Limits of this comparison
- Your doses are your own. The standard cycle is the White House's illustration, not a prescription; total FSH in a real cycle can be much lower or higher.
- Posted prices are not the whole price. Mandell's calls its prices estimates and adds a service fee at checkout; Cost Plus Drugs adds shipping ($55 for refrigerated items). Retail figures are Drugs.com's cash prices, and actual pharmacies vary.
- Most fertility pharmacies do not post prices, so a lower quote may exist that no one publishes. We list the sellers we checked and found no price on each drug's page, so nobody needs to check them again.
- Prices change. Every figure here was read on October 4, 2026, and each drug page shows the date beside every price. We re-read every source each month.
