Frozen embryo transfers include up to four ultrasounds per transfer; additional ultrasounds are charged separately. Excluded services include stimulation and frozen-transfer medications, sperm cryopreservation or storage, embryo cryopreservation and long-term storage, blood work, preliminary evaluation or diagnostic testing, surgery needed to prepare for IVF, testicular or percutaneous sperm extraction, PGT-A/PGT-M biopsy and testing plus shipping, complications, pregnancy ultrasounds and physician visits, additional trial transfers, donor sperm, Zymot, Embryo Glue, and Calcium Ionophore. This package pricing is for self-pay patients without insurance coverage; insurance patients are billed fee-for-service and may owe deductibles, coinsurance, copays, or excluded services. Payment is due in full at the beginning of treatment. Cycle #1 must be completed within one year of retrieval. Transfers are provided only if embryos are available, with no refund for unused transfers or failed treatment.
